Dean attempts to drive them back home to the bunker, guilt tearing at him as he slowly loses the battle against both his guilty conscience and the invisible damage to his body. Sam knows that Dean is hiding something important from him, but what?

"Truth is, he’s winging it and faking it and hoping he makes it. If there was anything he could do to take back everything that culminated in the most important person in his life feeling like he’s not even on a "loved ones" list, he would, in a heartbeat. But they haven’t exactly made a card that read “Sorry I repeatedly left you and betrayed you and mistreated you, I do totally love you tho”. Distantly, Sam wonders if he’s the only human being on the planet to have fucked up this badly this way before, and sincerely hopes so."
Dean gets hit with a truth spell, at a moment where both lies and honesty have shaken his and Sam's relationship to the point neither feels safe and secure in their bond as brothers, let alone lovers. Will a dead witch with a chip on her shoulder succeed in breaking apart what neither heaven nor hell could separate?

At first Sam tries not to worry about it. Dean has been quiet off and on, since he took the Mark – since Gadreel, really – and now that it’s finally cured, he figures Big Brother just needs a little time.
But they're on a houseboat in the middle of nowhere, and Dean has stopped talking entirely. And Sam isn't heading back to shore until he fixes everything.

“One second,” Sam says, away from the microphone, talking to his perfect, smart, bloodless friend. “What’s up?” Sam asks, to Dean again.
What’s up?
Dean’s holding his guts in with a hoodie. He’s in a ditch on the side of the road, amidst miles and miles of farmland on the side of a highway Dean’s forgotten the number of. A werewolf had done it, so at least Dean gets to look up at a full moon as he dies. After Dad hadn’t picked up, Dean knew there was only one person he wanted to hear from. Only one person who mattered.
“Nothing much,” Dean says, a little strained as a throb of agony rolls up his body. “Thinking about hitting up a bar in a few and thought I’d check in.”
