YOUNG OAKS PRESCHOOL
Planting the seeds for success.
4 Month Evaluation
Evaluator: Mrs. Vienna
Relationship to Child: Teacher
Please score each area of child's development from a scale of 1-5 and provide a reason for each score. Milestones rubric can be found on page 2.
Academic: 3.5
Andrew is a bright toddler. He demonstrates strong observational skills and memory, often reminding me of planned events I have mentioned in weeks prior (a special visitor, etc.) His literacy is advanced for his age. His handwriting is meticulous. Once Andrew has heard a story, he can recite it perfectly. However, Andrew’s comprehension is lacking.
Social: 2
Although Andrew seeks social interaction by sitting near his peers and watching them, he does not seem to understand or connect with them or any teachers. Andrew is also disinterested in group activities. He is very easily upset if an item is taken away and does not share. Furthermore, Andrew exhibits extreme bouts of shyness. When asked questions, he only replies with one word answers, if he answers at all. I find this notable because he is very polite otherwise, always waiting his turn in line and returning items to their proper places during clean-up.
Emotional: 3
Andrew is attuned to the emotions of those around him and feels intensely. He does not do well with surprises or breaks in routine, but regulates when distracted with a task.
Motor: 3
Andrew is adept at climbing objects and getting stuck on top of them. On the ground, he walks slowly and carefully. He has some trouble gripping and holding objects.
Language: 1
Andrew rarely speaks, even when prompted. He does not express his wants and needs with words. He also does not acknowledge verbal commands.
Other Comments:
I recommend sending Andrew for a speech evaluation on his expressive and receptive language skills. It is possible he may have a hearing loss. Seeking guardian approval so that Andrew can meet with the resident psychologist and assess his social development. It is good to address these issues now to prevent going into special education in the future.
