Dad would push Sam to Dean’s arms every time and Dean would change his diaper without a word of complaint. It felt familiar back then. It reminded him of Mum.
At that age, Dean wanted to do everything Mum did for Sammy. It hurt him whenever he couldn’t do something. He had forgotten about it. The helplessness he felt when he couldn’t provide for Sam.
Looking back now, it was such a stupid feeling. Obviously, Dean couldn’t make milk for Sam. He wasn’t a girl. He was also too young to be able to do it anyway. God, Dean couldn’t be more thankful that nobody else knew about it. Sam didn’t remember and Dad never knew. Dean never let Dad see. He could feel Dad’s eyes following Dean around. He knew Dad saw someone else there. It made Dean just the tiniest bit happy to be able to make Dad smile but he couldn’t let Dad see how he failed at the one thing Mum could always do; make milk.
OR
Dean never gets over wanting to breastfeed Sam and being unable to do it. It escalates when Sam finds out.

Summary
There were no exits that Sam could reach in time, and he was going to hit a wall if he kept walking backwards. He stayed still as Rowena stopped in front of him, eyes gleaming. “Now, see, I already have a hamster at home…but you know what I don’t have yet, Sam Winchester?”
She whispered something in Latin, and things started going dark. Sam tried to fight it, clinging onto consciousness as he gasped and fell to his knees. But the spell was stronger, and he only heard two words from Rowena before passing out.
“A dog.”
(Rowena casts a spell to turn Sam into an obedient sex slave for her and Crowley. Dean has to deal with the aftermath.)
