He can’t handle losing Cas, and he especially can’t handle losing the ex-angel over something as stupid as him liking to dress up in pretty underwear because it makes him feel beautiful.
Dean will either never speak his mind or he will die of humiliation, and he far easier will take the former.
(TLDR: Dean Winchester and his macho-manly internalized shame struggles to accept the fact that he likes wearing panties. Cas shows him there's nothing wrong with it. Angst, some miscommunication, love, and smut ensue.)

Summary
His stomach plummets as he sees Dean’s big green eyes glitter and his lower lip tremble. “Oh, my dear, my Dean,” and Dean collapses into his arms just as he bursts into tears. He’s wheezing out big, broken sobs and clutches at Cas’ coat. “Shhh. It’s alright. I’ll make everything alright, my precious omega.” He cradles Dean, running a hand through his short, spiky hair and pressing his lips to his forehead.
Unfortunately, Dean bursting into tears is not uncommon these days. His hormones have been all over the place, and it's not unusual to see Dean crying over mismatched socks one moment and horny as all get-out the next. Cas never minds the rollercoaster of emotions; he knows it’s a lot harder on Dean than it is on him, and he’s carrying their child, for gods sake.
This doesn’t feel like a mismatched socks moment, though. “Tell me what’s going through your head, my love. You're angry at that man?”

Summary
This is a re-work of Season 12 moving forward. I always thought it was a mistake to bring Mary back. Dean never needed to look back in that sense. She never treated him like a child or helped heal the broken inner one. So I always thought, he should have been able to be happy and be a Dad.
I tried really hard to keep the cannon of everything that happened before the start of this work. But with that said, some stuff might just be wrong. I tried, I'm only human. I did my best but I'm sure I've missed a few things. I have read so many amazing works on here that I think some things from those have stuck in my brain and are now cannon =) Whoops.
