The child stares at him with wide, frightened eyes, upset by the presence of an unmated, unfamiliar alpha standing close to her mother. She clings harder to Madara and buries her face in his body. Madara pats her back soothingly as he narrows his eyes, his spicy-sweet scent of woodsmoke and freshly sliced ginger gathering protectively around the babe.

Summary
After his second death in canon, Madara is sent back in time to a year after the village’s founding. To his surprise, he stays in the village.
Hashirama raises his cup. Sighing, Madara raises his cup in turn and clinks it against Hashirama’s. Both of them sip their sake, and Madara places his cup down, giving Hashirama a considering look. Then, Madara leans forward to rest his head on Hashirama’s shoulder, and he smothers a smirk at the sudden spike he feels in Hashirama’s chakra.
With a moment’s hesitation, Hashirama wraps his arm around Madara.
“Come home with me?” Hashirama asks.
From where he’s smushed against Hashirama’s shoulder, Madara gives a slight nod.

The absence of heaven is hell.
One year after Hashirama watches Madara die in his arms from Hanahaki Disease, he’s transported back in time to the founding of the village. Somehow, his second chance ends worse than his first.
Hashirama feels his gaze grow faraway as he remembers that fateful day. “I watched you die violently in my arms,” he whispers. “You told me you loved me. How could you put me through that if you loved me?”

“Because the alternative would have been much, much worse,” Madara says, his voice clear and filled with conviction. “I did it because I loved you. If I hadn’t, you would have lost faith in the village. I would have jeopardized the future of the village, and I couldn’t bear to tear away the one good thing left in your life.”

In the effect and cause, the punishment and sin, Hashirama gets sent back in time and forces Madara to undergo the surgery. (It goes as well as you'd expect.)
In and all my pleasures are like yesterday, Hashirama gets sent back in time once more and decides he has had enough. Third time's the charm, right? (Somehow, it goes even worse than his first two lifetimes.)

Summary
Izuna was a minor character in the novel. He barely had any screen-time and mostly existed as a plot device. His death spurred Madara to eventually forsake the village entirely and turn against them all. His death caused an unfathomable rift between Madara, Hashirama, and Tobirama himself. Hashirama no longer trusted his brother like he had before. Madara’s loathing of Tobirama represented all he hated about the village itself. Tobirama could not get over his fear of Madara, and Hashirama’s affection for him, and it poisoend the village for decades to come.
Tobirama doesn’t particularly want to repeat any of that.

“Remind me,” he finally said, making sure to let a tiny bit of annoyance shine through his tone. Maybe he could just trick them this way— “What exactly is your mission again?”
Both men hesitated for a second, then the older one spoke up. “One: Keep an eye on Uchiha Izuna at all times. Two: Make sure he does not ever notice your presence. Three: Every injury he sustains on your watch will be inflicted on you as well. Addendum: In the occasion of him getting undressed or changed, watching him do so is strictly prohibited. Violations of this order will result in the forced removal of your eyes.”
