It’s the same tongue Satoru has seen countless times in his own delusional fantasies, so vividly that he feels like he already knows it—though its only purpose so far has been to rile him up.
Geto Suguru may not be the danger or the problem the school perceives him to be, but to Satoru, he’s definitely the biggest problem of all.
Satoru and the somewhat friendly delinquent of the school have been dancing around each other for months. Satoru thinks it's finally time to act on their desires. Fortunately for him, Suguru thinks just the same.

The argument has been going for eleven minutes.
This is not unusual.
What is unusual is the location—the corridor outside OR three is not, technically, a designated space for interdepartmental disputes. There is a process for that. There is a form. The form is a single page, double-sided, and it asks the parties to describe the nature of the conflict in neutral language and to identify three potential resolutions.
Dr. Geto and Dr. Gojo have never filled out this form.
The administrator who handles this form stopped expecting them to at around the four-month mark, approximately six years ago.
or: Geto and Gojo work in the same hospital. This is, according to most of the staff, a mistake.

They meet each other a few years later, and it's not the same, but it's not completely different either.

“Yeah. You too, Mei.” He hangs up, tosses the phone from one hand to the other, and stands a moment in the centre of the foyer. Suguru thinks for a moment that he’s waiting for a round of applause, or perhaps an Oscar delivered on a silver platter.
Keen to stay far away from the train wreck that is Gojo Satoru, Suguru keeps moving. His shoes squeak obnoxiously as they make contact with the polished floor, and he tries to focus on that instead. It doesn’t work, and the further he gets from Satoru, the more his brain seems to lag behind.
Momentarily, he betrays his own judgement and risks a glance over his shoulder, just as Satoru is rolling his head back, staring at the ornate plaster of the ceiling. The way he moves, even when no one is watching, is like his body obeys rules that no one taught him out loud. Suguru despises that he notices. He despises, even more, that the noticing feels like being shocked by a fully-amped fucking taser.///OR: Geto Suguru, a housekeepers son, moves into the Prime Minister’s estate. “Breathe in,” Suguru said, and his voice was so low it was scraping against the husk at the bottom of his register. He was counting, but the water crashing in Satoru’s ears was drowning it out. “Breathe out,” he said, and then a horrific pain splintered across Satoru’s chest.
Even his addled state, Satoru could hear the sound he let out, some amalgamation of a cry and a whimper, as he felt the needle sluice through each individual layer of skin. The bar was hot and heavy in his body as Suguru went to grab the jewelry and put it in.
And the worst part is, it felt incredible. The synapses were firing all over Satoru’s brain, sending a shock up his spine and shivers all over his body, suddenly hot to the tips of his toes. Suguru replaced the needle with the jewelry and the tiny moment of re-insertion felt like Satoru might be able to come on the spot.
He whined, a little, and hoped desperately it sounded like pain.

Gojo Satoru doesn’t handle pain well. Or at all, really. But Suguru is just so nice with his clients, takes care of them so well, and Satoru wants to know what that feels like. Sue him.
Basically, Gojo finds out he has a pain kink and takes Suguru along for the ride.
