Sam frowns. “I—we were in the car. We were headed to work on that witch case I think? That’s—that’s it, though.”
Dean studies him, as if trying to determine if he’s lying, and Sam fidgets under the scrutiny, knowing there’s nothing else he can say. Finally, Dean nods, and there’s a heaviness to his expression that Sam can’t make sense of. “You got whammied. Just—” Dean holds up one hand and whistles as he flops it down, as if to mime Sam collapsing. Sam winces. “Took me a bit of time, but I got it figured out.”

Summary
Standing in front of the bathroom mirror, he removed the tissue to get a better look. Fortunately, the cuts appeared to be shallow, the bleeding only a sluggish ooze. Then he leaned in closer when he realized the lines weren’t just random slashes. They formed discernable letters, much like the crude initials Sam and Dean had scratched into the Impala with a knife when they were kids. The etchings in his skin were more than just initials though; there was no mistaking the word the letters spelled out backwards in the mirror.
FREAK.
Bobby. Castiel. Dean. One by one, the members of Team Free Will have had their spirits broken in their quest to stop the Apocalypse.
It's Sam's turn.

Dean knows Sam better than he knows himself.
He could put his life on that fact.
Hell, he’s changed his diapers. Cleaned his cut knees. Taught him to drive a car. Frankly, Dean has been a major part of every stage of Sam’s life. He could pass a degree in Sam without breaking a sweat. Good, bad, everything in between.
One thing he can’t do is read Sam’s mind. More than not, he wishes he could. Peal away the masks and smiles and see what his baby brother is truly thinking from one moment to the next. What is happening in that big brain of his, Dean will never fully understand.

Dean and Sam face some issues that Sam has been battling with for a while.... Set around Season 9 (spoilers if you haven't watched that far) after Gadreel. Canon Diversion- No Mark of Cain. They sort their issues out a little instead.

"Dean? Wha-" And then Sam saw it. In a particularly large shard of mirror, his reflection stared back at him. But it wasn't his. Looking back at him were his expression, hair, and clothes, but someone else's eyes. Instead of Sam's greenish kaleidoscopic eyes, yellow ones looked back at him.
"You addict bitch." Dean snarled. "You just couldn't help yourself, could you!"
