They got Cas back. And everything should be perfect. Except it isn’t. Dean is still a complete mess, and he was so drunk when Cas came back that he thought he made it up. But Cas is still there the next day. And since Dean’s a mature adult with numerous apocalypses under his belt, he handles the situation by entirely avoiding Cas.

After defeating the Leviathan, Sam can’t find Dean. But he tries everything. He hardly sleeps these days. All he can think about is Dean. Sam doesn’t remember the last time he's been this alone. Even when he went to Stanford, he knew he could always call Dean if things really got bad. But now Dean’s gone. And most likely in danger. And Sam can’t do anything about it. He hates it. He hates it so much. So he buries himself in research, but it does nothing. Without any new information to help him, he keeps going in circles. Sam is afraid if he stops, he won’t be able to start again. And he can’t do that. Not to Dean. Dean would never rest until Sam would be found. So that’s exactly what Sam is going to do.

Plus, he’s re-written his own memories before like when he lost Cas in Purgatory all those years ago. It totally makes sense he’d make up a love confession to cope with Cas leaving him again. Dean’s suspicions are confirmed when Cas comes back and doesn’t say anything about it to Dean. And things go back to normal as if it never happened. Because it didn’t.
