“You can’t.” There was something horribly broken in Tubbo’s voice. “You don’t know what it’s like, but I do. You don’t want to know what would happen if they got their hands on you. Death would be better.”
“Then why are you going, then? Why can you risk that and not me? I’ll- I’ll- I’ll punch ‘em to death! I can protect you if you let me!”
“You don’t understand,” Tubbo said, pained frustration evident. “You’re a human. I’m one of them. To them, I’m kin, and you’re a thing to toy with. I hate it, but it’s my home. And this world is yours.”

Tubbo is a changeling who must leave back to the land of Faerie, his home. Tommy, of course, follows him. (He has plenty of time to regret it).

That’s what Dream says, at least, and Tommy’s learnt it’s so much easier to just go along with what Dream says. He’d grown tired of fighting even before his hair had faded fully white. He wasn’t even sure why he ever bothered. Dream was his friend- his older brother in all but blood, really- and he’d always been. He’d reassured him of that over and over again when he couldn’t help but think through the Wrong Memories. Maybe back then he believed them.“
It’s been so, so long since Tommy and Dream abandoned their mortality. Rumours about the Goddess of Death lead them to a seemingly normal village, but the two of them find some eerily familiar faces…

Summary
Tommy's life hasn't really been the best.
Having a dead brother whose ghost haunts him regularly, a random green guy who decided that he was going to abuse him daily, a shitty and neglectful family, people who used to love him now hating him, is quite troubling for his mental health as an average human.
So he wants peace. In the form of dying by suicide. But, like anything good he's ever had, it doesn't work. At all. It also leaves him in the worst possible state ever - limping helplessly and bloody in the frigid, icy cold.
And, after that pitiful attempt and everything that follows after, he has ended up being hunted by people. Along with his infamous tormentor, who seems to have had a change of mind. By a lot. Like to the point he's overprotective.
It leaves Tommy confused, but he's content enough with it. And then chaos with his torturer ensues.
or,
Tommy tries to commit suicide, and ends up in a manhunt, allied with Dream while being hunted by George, Sapnap and Bad. Even after the manhunt, Tommy still has to face the Dream who he's learned to hate, but now he does it with support.

Summary
"Oh." Dream takes note of the look of fear in Tommy's eyes. Somehow he feels that he should be guilty of this. And to think that he'd never thought of the day he'd get to see Tommyinnit dressed in rags and eyes full of fear. It was so unlike Tommy.
The Tommy he knew was loud and brash. He was energetic and brave, known to bring chaos everywhere he went. This Tommy however had been more quiet, more threatening and with dull and sunken eyes that never show emotion unless it had been fear.
Dream's heart sinks at the sight.
