Summary
Dean noticed right away. Of course he did. He knew how to read every minute facial expression and twitch that Sam had. He had a lifetime of practice. He could recognize without a thought the way Sam tilted his head when he was angry and trying not to lash out, and he knew how Sam would clasp and unclasp his fingers when he was nervous. And Dean especially knew how to recognize the way Sam would pinch his lips tightly together when he was trying not to grin, but had definitely found Dean’s joke funny.
So when Sam flinched at Dean’s grip on his shoulder, Dean noticed.

Summary
It wasn’t long ago that John thought that Dean was a lost cause. The man was the one that constructed the whole situation: Dean was the bait, Dean would stay still, nothing would go wrong. Of course, something went wrong. Murphy’s Law governed Dean’s life.
Dean was the next target of the skinwalker’s scheme, and one little bite from the thing caused a whole rift between Dean’s idea of family and Dean’s idea of his father. John didn’t see Dean get bit, but he knew something was off the moment that he killed the skinwalker and Dean looked aghast.
So, like a lame horse, John took Dean out to the woods, when Dean was delirious with fever and writhing in pain, and left him there. Dean really wanted to say he was glad that John didn’t shoot him, but that would be a lie.

Summary
Somehow it wasn’t enough. Breaking his hand wasn’t enough. Throwing up everything he choked down wasn’t enough. And beating on his bulging stomach while it cramped and gurgled violently from the abuse still wasn’t enough to make his fucked up brain convince itself he was no longer trapped inside Alastair's world of infinite agony.
Or Dean eats his weight in empty calories and throws it all up again because he needs to remind himself he is alive by breaking himself apart.
