After the events of Thriller Bark Sanji unexpectedly runs into Ace on a small island.
The young commander can tell that something is on Sanji's mind right away and asks him about it.
After Sanji confides in him over dinner Ace offers him to spend the night together to distract him a bit.(just fyi, i purposefully left out some tags to not spoil any minor details that i thought coul be cool so yeah, enjoy!)

He shouldn't care, he shouldn't because he finally had Ace where he'd wanted him all night but he had strict rules when it came to his workspace. And if their little make-out session turned as spicy as Sanji hoped it would they would mess it up at least, break something at worst.He expected Ace to roll his eyes but he simply picked him up again and crossed the room, his new destination obviously being the big dining table.
“Wait, no, I haven't-”
Sanji didn't get to finish his sentence because in one swift motion Ace had him laid down on the wooden tabletop and leaned over him with a cheeky grin.
“Shut up and kiss me."

Sanji is thrilled to see him again and it's obvious that Ace feels the same way about him.

For a long moment the cook simply continued to puff away on his cigarette while Zoro glowered at him, but finally he said, “Maybe this wasn’t such a good idea. You might be able to keep yourself from touching me, but that means I’ll have to ask you to move around if the rest of this is going to work. And we both know how you feel about taking orders from me.”
“Says who? I can take orders from you just fine, asshole, as long as they aren’t stupid.”
It was only once the words left his mouth that Zoro’s brain actually registered what he’d just said, and he became painfully aware of the fact that he’d fallen into a trap. What trap he wasn’t sure, because Sanji’s ‘challenge’ to keep his hands to himself still didn’t seem all that difficult, but he could tell from the way the cook’s eyes lit up that Zoro had done exactly what he’d wanted him to. Goddamnit.
Sanji issues Zoro a challenge to keep his hands to himself during sex. This proves to be a lot harder and a lot more enlightening than Zoro anticipated.

“I’m only doing this because I don’t want to wake up Chopper,” he announces, glaring at Zoro as he climbs into the nest. “So shut up and sit down so I can get this over with.”
Zoro groans when he sees Sanji. “I thought you were leaving.”
“I wish I was, but because you’re a stubborn idiot, I can’t,” Sanji hisses, pointing at the bench and waving the kit around. “Sit.”
Zoro eyes the kit, then looks at Sanji. “I can do it—”
“Sit. Down.” Sanji feels like he’s about to crawl out of his body, irritation buzzing so intensely under his skin he’s shocked it hasn’t shot out through him. He hates this. He hates seeing Zoro like this, hates that Zoro doesn’t get it, hates that Zoro won’t listen.

Or: Sanji didn't try to sacrifice himself because he cares about Zoro, it's because he cares about the crew.

Summary
An onslaught of medical jargon Sanji can’t make sense of continues from Chopper, but he’s able to make out the word “surgery,” which is enough to send a fresh wave of nausea through him. Robin is performing CPR, blood oozing out of Zoro’s chest with each compression and blood smearing on her chin from each rescue breath.
Nami slaps Sanji’s face.
It’s so familiar and jarringly associated with his affection for her in his head that his distraction breaks completely. The hurricane around him mutes and he can only see Nami in the eye of the storm.
“GET A FUCKING GRIP, SANJI!” She roars. “I don’t know what to do when someone’s going into shock and Chopper is busy!”

Or, post-Thriller Bark, Sanji and Zoro reckon with what happened while being so mad at each other and themselves that they might just have to kiss about it.

“Not like what?” And Dazai loathes the way that Kunikida says those words, all soft around the edges like he’s talking to some traumatized civilian who got caught up in something they weren’t supposed to.
“You know what I mean,” Dazai insists, pushing himself up to stand at his full height. “I’m not like that.”
Not human. Not built so softly that he was traumatized by his upbringing. Not made of glass or porcelain. Not a thing to be handled delicately. Not someone who was undeserving of what came upon him.

5+1 of Dazai learning that growing up in the mafia does in fact have some adverse effects and no, supressing your trauma does not make it go away.
