And i realized that, when all this was over, I'd be left alone, barely breathing and completely broken. I'd be sad and miserable. It'd be so painfull that i might never be able to enjoy life again.

This story starts during "The Reichenbach Fall" arc of Season 2 with references that spoil Season 4.

Summary
When Isabelle Holmes stumbles upon her husband on one of her few outings to Marylebone, she recognizes the man he's with as John Watson. Upon realizing John Watson doesn't know Mycroft has a wife, she realizes that something will have to change. For seven years, she has been willing to play down their relationship around his colleagues, letting everyone assume she'd the closest thing to a friend he can keep, because she assumed the small circle he seemed to know through Sherlock knew. Knowing John Watson doesn't even know about her makes her reevaluate what she is willing to accept. The question then becomes if Mycroft can reevaluate his priorities.

Summary
When Sherlock told John Watson that he didn’t have any friends, he only had one, he wasn’t lying. Aurella McCoy wasn’t a friend, she was his sister-in-law. His brother simply hadn’t formalized it yet. Or even caught on that he had his “goldfish” at all. Sherlock really needed to do something about that. After he figured out how to get John to call him his partner in a more formal sense. Priorities after all.
