Summary
He parks next to a number of cheap sedans that must belong to underpaid teachers. With a few simple directions from the front office, which gives him side eye for his past dismissiveness of Megumi’s antics, he finds the propped open door with a red cross on its front. He knocks as he steps inside.
“Mr. Gojo?” The nurse looks up from her game of solitaire. Megumi’s sitting tensely in a chair positioned near her desk in a way that says she wants to keep an eye on him. He’s staring intensely at his lap, gripping the fabric of his pants so hard his knuckles are white. His elbows are locked straight, like they’re fighting the hunched shoulders trying to curl him in on himself. He’s pale, quivering minutely.
“Yeah, yeah,” Gojo vaguely acknowledges her as he rounds the few cots intended for the students Judgy Jane decides are actually ill. He ruffles Megumi’s hair. “You feel like getting out of here?”
No response. He doesn’t even look up, just inhales with a quiet sniff.

Megumi doesn't handle Tsumiki's coma well. Gojo does his best to deal with him.

Summary
He takes in a heavy breath and pretends these cheap sunglasses hide his dark circles as they wheel the poor kid in.
"Hey," Gojo rasps. "Miss me?"
Megumi's eyes go wide in horror. His jaw hangs slightly agape. He's breathing too fast. He doesn't say a word.
“Fushiguro?” Yuji's hand hovers over his shoulder. Megumi smacks it away harshly.
He trembles as he gets to his feet, then trips unceremoniously toward the foot of Gojo's hospital bed. Satoru instinctively reaches out to catch him, but he's too slow in his sewn-up state, and Yuta gets there first.
There's tears running down the kid's face. Guilt is a weighty boulder between the two of them.
Megumi claps his shaking hand over his mouth, but he can’t contain an emotional dry heave. Yuta escorts him to the trashcan so he can sob and vomit freely in terror.
Gojo sits there stupidly, unable to do anything else.

Gojo lives to pick up the pieces of Megumi. And there's a lot to pick up.

Summary
Now that he's looking for it, Yuuji can feel it: the words are etched into him, woven into his muscles and carved into his bones. Start by saving me, Itadori. Yuuji promised he would, so now he must.
The burning pain is starting to come back again. It begins in his chest this time, slightly to the right. Yuuji swallows, reaching up to press his hand against it.
So, he thinks dazedly, as the universe begins to take him apart. This is what happens when you break a Binding Vow.

Megumi dies in Shinjuku. Yuuji is thrown back in time to save him. Again, and again, and again.
(Or: Yuuji says “I’ll save you, Fushiguro” and the universe says “bet.”)
