Summary
He couldn't "feel" what you call "emotions". Normally children will be full of them as they easily express themselves but not for Douma. Although, he has heard of them, he doesn't know what it's like to have them. Or even understand them. He doesn't try, he doesn't see the value of them. He pretends to have them with false personality. He doesn't receive what you call "affections" from his own parents. Though he doesn't care. They're too busy turning him a religious leader of the Paradise cult. They believed he can hear the voice of Gods, so making him be leader matters. They're making him carry such responsibilities of hearing people's burden. He doesn't mind. He just has to pretend to even care about his followers' boring sob stories.
"Once they die, it's all just gonna return to nothing. They won't feel anything."
That's what he truly believes.
"And then they rot into the ground. As long as you live on this Earth, that's your eternal fate."
But he believes he could help them. Make them happy.
He also met a pretty but naive girl by the river in the deep woods. He honestly thought she was pretty stupid. But. He couldn't help to enjoyed being around her. To the point, he actually feels. -

That is, until he meets her: a lovely, petite, and bright girl named Koyuki. Her beautiful name, containing the kanji for "love" (恋, ko) and "snow" (雪, yuki), seems to perfectly describe her.
A year his junior, Koyuki is kind, soft-spoken, socially awkward, and incredibly timid, with an innocent appearance that could fool anyone.
Or so he thought.

Kurusu stood up from her chair. “Will we be able to save Fushiguro by then?” she asked. Maybe it was the light from her halo that made her eyes sparkle. Maybe it was the full force of her naivety showing through the only way it could, or the way her hope forced its way through the spreading fog of doubt. Whatever it was, Yuuji was drawn to it. He smiled at her and caught a hint of her sweet optimism. “We’re saving Fushiguro!”
Between November 19th, when Gojo is released from the Prison Realm, and December 24th, when his and Sukuna's highly anticipated fight will take place, Yuuji struggles to come to terms with the loss of his friends and the seeming end of the world. By his side, whether he likes it or not, is Hana Kurusu, who is limited by her missing arm and inexperience, but not in her belief that he will rescue Fushiguro.

Her days eventually seamlessly merge into one cycle of carnage, and searching aimlessly for a mythical flower. If only she'd knew how it would unfold from there.
