Annabeth saved the Athena Parthenos, and her friends and boyfriend find her under the city of Rome. As they're securing the ancient statue in the ship, Percy falls over the edge of the collapsing cavern, and while trying to save him, Nico and Jason go over the edge with him. Together, the children of the Big Three fall into Tartarus, and Wisdom's daughter must lead the rest of the crew of the Argo II to the House of Hades in order to save them and defeat Gaia once and for all.

He kept his eyes open, staring at the wall, so consumed with guilt and flashbacks and pain that he couldn’t muster up the courage to close his eyes and even have a chance to be plagued by those dreadful nightmares. He laid there on his bed, listening to the faint screams of Annabeth from the other room, Piper bursting inside to comfort her. None of the the rest of the Seven took note of Percy's eyebags the following days.”

His mental health is at an all time low, his body is starting to shut down thanks to his lack of appetite, even when Will tries to get him to eat more. will seems to have more issues than expected and he realized that they're more alike than he thought.
Eventually, Nico realizes his feelings for Will and he has no idea what to do, so he goes to his other friends and Will's siblings for help, not realizing Will had done the exact same about him.--

A drop of red coloured the sheet below Nico’s mouth as he hunched forward. More quickly followed and blood began spattering the clean white fabric.
“Nico!” Will’s hands were shaking again as he placed a hand on Nico’s shoulder, watching in horror as blood kept falling in constant splatters from the son of Hades’ mouth.
“No, no, no, no,” Will found himself repeating as a few drops of blood found their way from Nico’s mouth to the back of his hand.
