Dean could tell that Sam’s hallucinations were getting worse but he didn’t know that the hand trick wasn't working anymore. Unfortunately he finds that out when coming back to the motel with Sam carving the wound back into his hand with a knife.
Sam doesn’t even understand why it bothers Dean so much. For some reason that makes it so much worse.

Sometimes, when Sam dreams, he wakes up to Dean looking at him funny.
“What?” Sam asks, sometimes, rubbing his eyes. He always dreams when he sleeps now. The cage doesn’t drive him crazy anymore, but it does fill his nights.
“You were talking in your sleep, man.” Dean sometimes says. Sometimes he just shakes his head at Sam. Sometimes he just asks if Sam’s okay. “Something ‘bout, I don’t know- I think I heard the word ‘burn’?”
Sam nods. He only dreams in Enochian nowadays. “Yeah.”

Cas wasn't able to heal Sam's hallucinations at the mental institution and, in a last ditch effort to do something for Sam, Dean breaks him out and brings him back to the bunker. But there just isn't a cure for the sleep deprivation because more so than his mind, Sam's own soul is what's eating him away from the inside. That's just what happens when you throw a soul into Lucifer's dog bowl.
OR
It's been 15 days since Sam had last slept and Dean finally gave up on researching for an answer and decided to stay with his baby brother in the end.
