Aizawa got (debatebly) lucky with getting a narcolepsy diagnosis early and intervention and medications and sleep studies and accommodations quickly followed. Aizawa's lived with this body for years, lived with narcolespy for years. He's used to it, it's his normal. Having narcolepsy is just another natural part of his life in the same way that being a pro hero is part of his life, that being a teacher is part of his life, that being a husband is part of his life.
That doesn't mean that sometimes it still doesn't blindside him.

The thought needles at Katsuki's mind. How much pressure would it take until his face starts turning mottled red? How close was he to death all those times? Once the idea is lodged in his chest, it won’t leave, and Izuku has always been a haven for him to unload his weakness. He would never deny him anything, no matter how far Katsuki pushes him.
He tightens his knees by Izuku's hips and leans down, sweaty hair falling over his eyes. His heart throbs in his throat, wild and frantic. "I'm gonna choke you."
That's when he realizes Izuku isn't going to stop him. Katsuki is going to choke him and Izuku is going to let him.
