"Mom, is there anyone else here? I heard someone say something,” Izuku asked, sitting back up in his chair, looking at Inko with innocent curiosity. She frowned in confusion, she didn’t hear anything.
“No? It’s just the two of us here, maybe it was just the neighbours?” She responded softly. Izuku looked as if he was about to agree and move on, when that hint of colour in his eyes suddenly engulfed half of their usual green. Izuku’s brow suddenly furrowed as he looked up at his mom.
“I’m not the neighbours.”

This wasn't how it was suppose to go. Dream felt that he has done everything to get through to Nightmare, but to no avail. But to resort to this? He didn't think things could have gone so south so fast. He just wanted his precious brother back. Was that too much to ask for?
It was an accident.

Summary
It wasn't on purpose. Not the first time anyways. It just slipped out, and Dream just went with it rather than tell the child that he lied to them. Who didn't want to be literally anyone else anyways? Besides, Blue said he needed a break, and this is how he's getting one.
OR
Dream accidently invents a new persona. Normally, this would not be an issue. However, due to various causes for mental instability, such as stress, trauma, and a complete and total lack of healthy boundaries, this ended up as some sort of coping method.
