Summary
Muggles set off a catastrophic reaction of ley lines, leading the entire wizarding world and other magical creatures to pack up and abandon Earth for a new world. Now they are settling on a new world where the flora and fauna are oversized and the apex species is massive, intelligent, telepathic dragons. As the only person able to travel through the portal unshielded and bring everyone and everything across in habitats, Harry is the most famous person on Omen, much to his disgust. As a bonus, though, Rita Skeeter is forever relegated to writing obituaries.
This will make moderately more sense if you start with the first fic in the series.

The Book of Potter. Whereas the Family Grimoire contains everything that made up the mechanics behind the Potter magic; the Book of Potter connected the people within that family, no matter how far back in time. With it, Harry (and anyone else keyed in) can communicate with any Potter by blood or marriage to get their insights on what makes a Potter, a Potter.
Prior to that fateful Halloween in 1981; James and Lily Potter decide to change the script that Fate had decreed for them and invited Lily's sister and brother-in-law to go over some information should the worst come to happen and they were unable to care for Harry. Because of the Book, the Potters were able to escape being killed by Voldemort and only rendered unconscious. Harry was still dumped at the Dursleys by Albus, but the mood had changed. Petunia and Vernon didn't resent Harry because the letter that they should've read was lost.
Lots of changes in the timeline resulted after that misplaced letter...

Dumbledore tried very hard not to lose control, but the situation was serious, and he could not involve either the Order – which had been disbanded several years earlier – or the Ministry – which was not exactly involved or aware that he had arbitrarily, without any consultation or verification with the distant magical family – taken and moved the Potter baby after the death of his parents.
Harry Potter had disappeared from his aunt's house. The Dursleys had all been Obliviated. Harry Potter had vanished into thin air.
In fact, technically speaking, he no longer even existed.
