Despite Kotaro's best attempt, Sakura figures him out. She knows he's Inui. It's pretty obvious when he draws the exact same as her old friend. It takes her an insanely long time to figure him out. A whole one week after her memories return. But after she sees through his secrets she decides to go along with the lies for his sake, because she for the life of herself can not figure out why he would do all this and have this huge identity change. Resurrecting your friend and making her dreams a reality is pretty kind and a sign of a wonderfully pure hearted friendship! Why wouldn't he want to own up to that?
Over time as this huge confession that she's waiting for never happens and she gets more and more impatient, it starts occurring to her that Inui's feelings might not have been as platonic for her as she thought and there might be a reason he's creating these lies and distance for her sake.
