Ianto doesn't die during the confrontation with Syriath. By all accounts, that should mean a happy ending for both Ianto and Jack.
Pity that they're from Torchwood, where happy endings are (almost) non-existent.

Summary
When a wish is made, the universe holds its breath. Clear, concise, with carefully picked words. That's how a wish needs to be made. However, most are screamed into the ether without thought, fueled by desperation and passion and fear. Final moments, patience run thin, too tired to carry on . . . It lulls people into a false sense of security. And in those instances of hopelessness, when no other option appears viable, someone takes a deep breath and speaks toward the sky.
But instead of divine providence, they grasp ahold of a Monkey's Paw.
In 2009, Captain Jack Harkness made a wish. And that wish has haunted him ever since.

Summary
Then the noise and the wind stopped, and a blue box stood in front of him. Before Owen could even begin to think, the box’s door flung inwards and a haggard man slumped against the frame. An eerie red glow lit him from behind, but Owen couldn’t see past him to make anything out.
The man raised some sort of cylindrical device at Owen’s face. It buzzed and glowed blue at the end, and the comms piece in his ear sparked and popped and clattered, still fizzing, to the floor.
“Owen Harper?” the man panted. His voice was hoarse, scratched raw. He half stepped out of the box, one hand gripping the frame for dear life while the other trembled as it reached out for him. Palm up. Like a welcome. “Come with me.”

He'll have to delve into some dark powers not seen since the days of the Time War (apart from that one time) in order to assemble a team that can take on this threat. But will Torchwood trust this mad man in a box?

Her first thought was that it was not Jack Harkness. His height still surpassed her’s and this person was far smaller. Her second thought was to look down, and she did, quickly spotting a mop of blond hair and a bright sunny grin, smiling up at her.
No - It couldn’t be. He was gone. But as she watched and he continued to grin up at her she was forced to admit it couldn’t be anyone else.

The twenty-first century is when everything changes.
Jack Harkness had known that for years - it was one of the first things that he had learned when he joined the Time Agency. He just never thought that he and his husband would be helping to lead those changes. Or that their adopted daughter would have such a huge role to play in the entire thing.

Summary
The Nethersphere was a device designed to hold the dead until such time as they can be downloaded into an army of desecrated Cybermen husks, at least if one had died before 2014.
Ianto Jones, Owen Harper and Toshiko Sato, former members of Torchwood 3, were dead by 2014.
