Tommy didn’t know what he thought the afterlife was going to be like, hell he didn’t even know if it was a real thing. Maybe he’d see Wilbur, possibly Schlatt, but he didn’t expect to see a white castle and Karl.
Or where Tommy looses his final life to Dream in the prison only to be teleported back in time.
OR where Tommy is given a second chance and isn't going to blow it, not even if things start to get revealed (things that change everything) and discoveries are made.

Tommyinnit, the famous Minecraft streamer, is minding his own business when he's suddenly transported into the dsmp storyline. After much thought, he decides to try his best to prevent wars and make everyone slightly less traumatised. Oh, and he also gets murder voices.

Summary
The particles around him accelerate as he turns around. Looking up to see Wilbur sprinting full speed towards him causes Tommy to choke on the sob trying to worm it’s way out knowing he almost did that earlier this evening. There’s a chance this night might break him, but right now? Right now he’s watching his older brother run toward him, and he pretends like Wil’s coming to take Tommy home.
Sidestepping behind a wall Tommy whispers, “Bye, Wilby.”
And when Wilbur turns the corner?
No one is there.

Or: Tommy's cursed to travel through time, meeting new friends and family, before being forcibly ripped away from them every time. He just didn't realize that those people are as weird as he is, and love him very very much.

A hand landed in his hair, playing with his golden curls, a rumbling close to his ear. The Blood God was so close, so close, so close, too close.
The Angel of Death crouched down in front of him, a concerned look in his stormy ocean eyes. He smiled softly at Tommy like he was the most precious thing in the world, and the boy couldn't even stop the warm feeling curling in his gut.
The man's taloned hands reached forward, gripping Tommy's chin between them and making blue eyes meet blue. "We can't," he murmured, like one would coax an injured animal. "No one's taking care of you, dove, not even yourself. You're here now, though. We'll take care of you."

Summary
To be strong in this world, is to have a target over your head forever. The second anyone thinks you're powerful, the government takes you away to never be seen again.
Here is a world where everyone is born with a unique power. This power is known as a major power and is discovered by kids when they turn 10. As they grow older some people learn to fully control and master their respected major power. But this is where the world twists and bleeds because the second you fully master this power, and anyone finds out, the government's police force, Enforcers, are sent to capture you, and never seen again. It doesn't matter if you are a 34 years old man or a scared 12 year old boy who's never felt safe in his life. The Enforcers will find and contain.
In Layman's terms, Tommy loses everything for his power. Philza tries to keep everyone safe, along with the family's new addition. Wilbur misses his mom. Techno swears to protect. This story follows all four as they try to live in a world that wants them gone.

“I think we know what’s wrong with you— and I think I know how to cure it,” Phil is trying his hardest to look encouraging, and reaches a tentative hand towards Tommy's shoulder. He halts, his fingers stilling in the air for a moment. Eventually, he sighs and withdraws his hand as if he were burned.
Tommy's scowl deepens. He doesn't trust Phil, not one bit.
“So? What do we do?” Tommy says, his tone aloof and stubborn. Phil gives him a pitying look, and Tommy doesn’t know what to make of it.
“We’re going home, Tommy.”
Flower Head is a fatal disease caused by extreme bouts of nostalgia and a longing for a past that is no longer possible. Flowers will begin to sprout from the scalp, slowly making their way to the eyes, face, and neck, blinding and eventually suffocating you.
Of course, Tommy falls victim to the illness.
The only cure? To bring him back to where his fondest memories lie.
It's time for their family to go home.
