Start with a heart surgery that goes as sideways as possible. Add two doses of Extremis, to Palladium, Starkanium, and nanobots and in Tony's case you get the beginnings of something Superior.
But, even a technopathic, pyrokinetic, Superior Tony Stark cannot account for everything.
In the SI Financial Department Grace Simons discovers both long term embezzlement and its connected cover-up. Though she's weary of the Superior side of Stark she adheres to her values and reports what she's found.
The guilty will pay.

“You’re going to regret it,” Tony said quietly. The beeping of the monitors had slowed, the panic had faded away leaving only a quiet exhaustion.
“I don’t think so.” And Strange sounded so sure.
Tony closed his eyes. He wanted to say no, wanted to let go of the weight of the world that had already crushed him. But he couldn’t do that. Couldn’t do that when Strange had used his last chance to save the world, banking on him. “Alright.” He swallowed hard, forcing himself to nod. “Okay. Send me back.”

After Thanos is defeated - thanks to the mysteriously missing Time stone, Tony can’t wait to dive into his retirement plans. He buys a nice little penthouse apartment, moves in his bot family and with the new Avengers taking care of business now, he can finally rest his weary body and haunted mind.
Powerful relics don’t just disappear into thin air though, and he soon finds out that the view and the piano are not the only things he bought along with his new home.
It’s not like realtors would come clean about annoying, medieval-looking ghosts that keep showing up, claiming this is their home, their piano and that they are most certainly not dead.
