Summary
Most, if not all, surgeries happened while the patient was asleep - knocked out cold by anesthetics so that the procedure could be performed smoothly and without disturbances. Dissections were performed on the dead, so one didn’t have to deal with the patient suddenly awaking in the middle of getting their stomach sliced open - although, Ford had experienced such a thing once, and after having to fight off the cadaver, he decided doing biopsies on zombies was a no-go from there on out. Vivisections, on the other hand, were quite like a mixture of the two - a twisted child of a surgery and a dissection - the act of taking someone apart while they were still alive, and trying to fit them back together again like nothing had happened.
But, unlike most participants of such a surgery - who lay unconscious, put under by drugs that will keep them still and calm and completely unknowing of what is happening to their bodies - Bill Cipher had been wide awake throughout the entire process.
And he was laughing about it.
Or Ford performs several nasty vivisections on Bill much to the dismay of everyone else
