James Bond had been dead for a week when the next one is brought to Q's office. Q had a relationship with the last James Bond and isn't handling it well.
In Casino Royale (1967) style, the name James Bond is just as much a code name as 007.
I try to always refer to the previous James Bond as James Bond or James and the new one as 007 or the agent. But things get... muddy.

Unfortunately for the sanctity of the window in the guest bathroom, the front entrance was too heavily fortified.
He slipped in, vaguely grunting as the action pulled at his barely scabbing wounds. His deerskin loafers made no sound as he crept forward, avoiding the subtle tripwires hidden carefully in the warm wooden flooring.
At last, he reached the master bedroom, and carefully turned the knob. The gentle pull to open it was nerve wracking, even for an agent as experienced as him.
There was an immediate flurry of movement, and his heart started to pound, his network of capillaries flooding with adrenaline as a sleek, modified glock was pointed at him.
It lowered a moment later. ”Bond?”

James wasn’t sure how he got away from the island. The island on which he should have died. On which he’d been willing to die. To save the only two people he loved. To save them from the danger he’d become.
It hadn’t been Madeleine’s voice which had saved him. The memories of the words that had made him leave the island in the end came back days later, when James slowly but surely was recovering from his several injuries.
Maybe it was Q’s insistent voice. He was used to listening to him, after all.
Or maybe it was Q’s little slip. I won’t let you leave me, he’d said.
It would take James a long time to come to the realization about what made him fight for his life in the end.After the events in No Time To Die, James leaves the island and saves his own life. He's devastated that he can never touch his family again. Never see them without being a danger. During his recovery, he's staying with Q. Her next words were burned into his memory forever. “They have Q, James. They’ve taken him and we can’t find him. We need your help.”During the events of Spectre, Q gets taken and nearly dies. Shaken by the events and afraid that something like this might happen again because of him, James leaves. He'd rather stay away from Q, even if it's killing him, than to be the reason for him being in danger. But when Q gets kidnapped again anyway, he returns to get him back. On his way back to London, James thinks of how he and Q got together in the first place and all the events that led to him leaving. - 00Q

Meanwhile, Q is tasked with the impossible, and must do everything that he can to ensure that Bond's health and future is maintained. The process leaves both of them exposed in ways they couldn't have predicted.

OP Summary: The man moved with the same predatory grace he always had, and if he had aged in the past five years (it was a bit hard to tell from a subpar camera on the other side of the street) then he had aged well. Which was all entirely predictable of course. No, what had thrown Q and everyone else in the ops center who had ever met Bond or read his file was the way that he was dressed - faded blue jeans with holes in the knees, a t-shirt from some unidentifiable but probably local surf shop, and-
“Are you wearing Converse?” demanded Q before he could think better of it.
Bond’s eyes unerringly found the camera through which Q was watching, an expression of vague displeasure. “Well, I don’t generally make a habit of wearing suits to fix my boat, and I doubted you had the patience for me to change.” .James Bond has been retired for five years. But when a highly sensitive mission goes to hell just a few miles from where he as settled down Q is forced to ask for his help. Bond and Q return easily to their old ways - they are also forced to finally confront their feelings for each other.
