Summary
Kacchan hummed as he lightly scratched at Izuku’s scalp and massaged the soap through.
Izuku braced himself against the wall. “Why are you doing this?” He sounded wounded—he felt wounded.
“Doing what?”
“Torturing me,” he croaked, resting his forehead against the cool shower tile and silently begging for his torment to end.
Kacchan was giving him glimpses of what he’d never have: careful touches, falling asleep in loving arms, being loved unconditionally, being seen for who he truly was, and being loved for who he was. And it was killing him.

Or, shortly after being discharged from the hospital, Izuku wakes Katsuki up with a bad migraine. Katsuki plays nurse as best he can with one functioning arm and a weak heart. As the days pass and Izuku fails to get better, Katsuki suspects there’s a deeper meaning behind his worsening condition.

Summary
"You're sick," Kacchan remarks.
"Yeah, no shit." The words leave Izuku's mouth before he can stop them. His muscles lock up, freezing him in place.
His eyes are wide as he stares at Kacchan, but even they can't decipher what's going on in his head. All Izuku knows is this; he really shouldn't have done that.
It's New Year's Eve and Izuku is sick. He's sick and alone and miserable. Then Kacchan is there, and he's not alone anymore.
And he's definitely not miserable, that's for sure.

The question takes Izuku so off guard that he doesn't answer immediately. His mouth drops open but no sound comes out.
"Who the hell names their kid Deku?" Katsuki continues, squinting at the papers he'd delivered.

Eyes darting around the room to a few curious looks. Bakugo leaned back, dropping his head on the back of the cushion and staring straight up, shoving hands back into his pockets. Midoriya pretended to refocus on his notes.

At some point, Izuku had to stop. Unable to help himself, his fingers loosened around the washcloth and his palm came to rest flat over Katsuki’s chest, right over his heart.
Thump Thump Thump, Katsuki's heart fluttered miraculously beneath his touch. It was a second chance Izuku didn’t deserve. His thumb hovered at the edge of his scar, where his pale, smooth, perfect skin gave way where evil had mercilessly shredded through. Before and after, separated by such an easy line.
He swallowed hard, his throat tightening around a cry, trying to ground himself in the fact that Katsuki was here, alive, that none of those scars had taken his Kacchan away from him. They only created new patterns on the beautiful body that Izuku would wash clean.

Summary
It hurts. To love Izuku.
Not because of the thorns digging in his throat, and not because of flowers blooming and taking up space in his lungs.
It's hurts because Katsuki knows if the nerd ever finds out, he would find a way. Find a way to love Katsuki, and to love him genuinely, with all his flaws and faults and would save him like the hero he is. Would do it, just to save his life. Would do it because Katsuki knows he still feels guilty for his first death and the second one being his fault would actually break him.
It hurts because any time Katsuki thinks he can get over it, Izuku does something like breathe next to him and he is gone once again.
It hurts. Because Katsuki can never let him find out.
It hurts because Izuku is so easy to fall in love with.
