In return, he gains Death's.
Or - Alec is a hospice nurse, Magnus is Death, and a lot of quotes in literature that probably weren't necessary.

Summary
Alec had always sort of known it would be his fate to die young one day. All shadowhunters lived with that knowledge. They went out each night fighting against demons, never knowing if this night might be the night they were that split second too slow, the demon just a little bit faster.
Becoming Institute Head lessened that chance, but it didn’t take it away entirely. Alec still went out into the field, he still fought when the need was great. He put himself at risk over and over again against demons, and against those who sought to destroy the world that he and his family were working so hard to bring about.
Never once had Alec imagined that he’d die while sitting safely within the comforts of his office, drinking from a bottle gifted to him by a friend.
Nor had he expected that his death would somehow prove to be only the beginning.

Given a second chance at immortal life, Magnus returns to New York only to discover that, despite the seemingly fleeting moments he’d spent in death, decades have passed in the mortal realm and the love of his life is gone. The only thing Alec left behind were letters, marking the passing of days: Day 3, Day 64, Day 378, Day 1104.
Through Alec’s words, the events after Magnus’ passing begin to unfold. And with each painful letter, a hopeful path back to the man who had the makings to be Magnus’ greatest love begins to be revealed.

That is, up until the moment Alec stood on the shores of Lake Lyn and faced the death of the one person who held a piece of his soul, and the lies that followed his mysterious resurrection.
There, on the shores of Lake Lyn, those strings finally pulled too hard, and Alec broke.
With the permission of the Inquisitor, and the help of the warlock who Alec had wanted so desperately to allow himself to fall for (and had been terrified to do more than smile at his flirting) Alec walked away from everything and everyone. He left New York behind and made himself a home in the small town of Prayer – a joke Magnus found particularly funny.
But, two years after that fateful night, Alec’s old life comes knocking, and those strings he thought he’d finally cut are tugging him home. Back to the place he never wanted to have to see again.
At least this time, he’s not facing it alone.
