Cas and Kevin have been living in the bunker for a few weeks post-angels-falling when Dean realizes it's time for him to go off his suppressants and have his first heat in a few years.
His suppressants weren't just keeping him from going into heat - he was on the extra-strength kind, the kind that let him pass for a beta if people aren't paying close attention and deaden his own sense of smell.
Which means that when he goes off of them, he and Castiel get a full dose of each other's scents for the very first time.

Summary
When Dean rescued Cas from the Empty, it should’ve been easy to yank Cas close and kiss him; making the confession a two-way street. But Dean was caught between too many emotions, and the moment slipped away. Days of silence bled into months, and Dean felt the distance between him and Cas growing into a chasm as Christmas fast approached. When Sam decides to meddle with some well placed mistletoe and claims that by the end of Christmas, they would be deanandcas again, Dean doesn’t believe it. The kisses don’t mean anything, it’s just mistletoe kisses. But, with each kiss, things start to change, and Dean begins to wonder if maybe these kisses do mean something after all.
OR
5 times Dean and Cas kiss under the mistletoe and it doesn’t mean anything, and 1 time they kiss and it definitely absolutely without a doubt means something.

Cas huffs and shakes his head, but he doesn’t seem to be interested in engaging in further discussion, so he just opens the book again and says without looking at him: “Good night, Dean.”
That’s all. Dean rolls his eyes and pushes himself up from the table, shoving the book a little rougher than necessary into the shelf and because he is a little shit and has to have the last word, he turns towards Cas again.
“So, I’m not getting a good night kiss then?”

When Dean discovers the ritual of Cas giving Jack a good night kiss, he teases the shit out of the angel. He doesn't expect for Cas to tease back. He also doesn't expect how much he likes it.
