The Duke has a pretty delicate ecosystem in his Fortress, but with the help of the inmates, he does a good job at making it a tightly-wound machine that operates smoothly. It runs like clockwork, you could say.

Summary
"You're smoking." it came out much like a statement, his delivery, steady. But the surprise, and perhaps, concealed disappointment in his voice does not escape Wriothesley's notice.
The duke admits, he did not take kindly to the Chief Justice's scrutiny when it came to his medicating habits. Possibly, it was due to how different their lives truly were, where he could not possibly understand why Wriothesley does the things he does.
Or
Wriothesley starts smoking again after being clean for a year, and people take notice very quickly.

Kunikuzushi removes the bandages from his injection site, taking with it some stray hairs that got stuck in the weak adhesive. Out from his thigh radiates masculine ichor, carried by linseed oil. For several beats beats, his blinks and pulse match pace, but the synchronicity falters when his heart relaxes. The few hours after taking testosterone are almost peaceful.
Through the brittle glass of his window comes the slow, mechanical drawl of an unmaintenanced car parallel parking. He doesn’t need to pull back the curtain to know she’s here.
The screen updates arrhythmically. Kunikuzushi looks to the anonymous messages that drift in like litter.
“Godspeed,” he says mockingly to himself.
With the help of a kind stranger, Kunikuzushi becomes a man.
