Shouta knows something's wrong, but doesn't realize exactly what is wrong until he sits Tsuyu down to talk, and only then realizes that the two of them are much more alike than he ever imagined. And he can't help but to wonder if Tsuyu's mindset and the trauma she's been through this year are his fault and a result of her taking after him.
