Summary
Something Mark never quite gets used to is how quickly patients can deteriorate. Just when you think you've pulled them from rock bottom, that alarm blares, and things get worse than you ever thought possible. It's jarring at the best of times.
It's even worse when it's your own fault.
OR
After being mistakenly treated for a relapse, Carter's illness is discovered, and the team fights to save his life as he deteriorates.

Summary
John had appeared a few more times in person, just two or three times for dinner, even more often in Peter’s stories - even once he’d dropped from surgery and was technically no longer her brother's mentee. Still, Jackie was relatively surprised when Peter arrived with a bandaged John in tow. Peter had swept into the home, as though it was routine for him to appear and leave bruised doctors standing in their entryway. He’d continued to busy himself, leaving John to stand there, awkwardly hunched, like his body weight was unfamiliar and causing him to sag.

A short selection of caretaking that happens after "blood ties" concludes. It's not necessary to read that first, but helpful.

Summary
The pain is all in his head.
When John was in rehab, he heard the words so often they became a mantra, one that echoed in his mind from the moment he woke up to the moment he fell asleep. Slowly but surely, the nonexistence of pain became an internalised truth.
Except now he's sitting on a bench outside of the ER, curled into himself because he hurts, and for once, he can't be convinced that the pain exists only in his head. Something is wrong.
OR
Shortly after Carter arrives back to work from rehab, he starts to feel unwell. He knows he hasn't been using, but does everyone else?

All throughout the procedure, Peter half expects Carter to burst in, panting, sweating, full of apologies and claiming some excuse: the El was delayed, he'd overslept, his apartment had burned down again. But Carter never shows.

Summary
“Dr Carter's… Dr Carter's been shot.”
Those are the words that crackle through County General's radio, but nobody truly believes them until the ambulance screeches to a halt outside the entrance, and Mark tugs open the passenger side door- only for Carter's unconscious body to list out, covered in blood from the neck down. Somebody sobs.
“I drove as fast as I could.” Zadro explains, stumbling over his words as the assembled team (Mark, Carol, Lydia, Malik) struggles to pull Carter onto a backboard. “Tried to put pressure on his neck but the blood just kept coming.”
OR
Carter is shot in S5 E17 instead of the other paramedic.

Summary
'Suicide' was a word tucked in soft quilt blankets and shoved in the deeper parts of his mind, even as he saw patients day-in, day-out who were outwardly, or in some capacity could be considered suicidal. [...] No one knows, never knew. Also, no one can be surprised. If they look further, they'll see just the extent of what he's hidden in plain sight from his childhood.

Carter has struggled with suicidal ideation for years. A night shift is almost his undoing. -

Summary
"John!" Michael caught his wrist, holding him still. John gave a tug but his brother only held on tighter. "This isn't about Pope, right?" He asked seriously.
"Fuck off, Michael," John managed to snatch it back, walls flying up instantly in defense. He did not want to talk about this, he'd been avoiding it and Pittsburgh for months.
"Come on," Michael laughed, exacerbated. "It was one week fucking months ago--did it really mean that much?"
"That week was the fucking happiest I have been in a long time, so yes Michael, it did really mean that much. Now if that's all, I'm fucking tired. Crash on the couch, bathrooms just off the kitchen."
"John-"
He pettily slammed the door shut, cutting Michael off as a final fuck you.

Summary
Standing with his back to the door is a behemoth of a man, grunting and snorting like a wild boar in a trap- and in the man’s grasp, pinned against the wall by the paws around his neck, is John Carter. Dead-eyed and slack as he dangles there like a marionette.
Mark's brain short circuits. He stumbles back. Yells for security, his own voice strangled, yet doesn't wait for the reinforcements to come, because the boy’s lips are tinged blue, and he isn't fighting like he should be.
OR
Carter gets strangled by a patient. Set in S1.

Summary
Set in S3. John Carter is hospitalized with a bad case of pneumonia. Eleanor Carter arrives at County General to sit at his bedside. Her visit, however, is motivated not by love for her ailing son, but by something far more selfish. Peter Benton comes to realize that his student's family life is not just complicated, but in fact deeply troubling.
Eleanor Carter didn’t ask questions about how John was doing, about his prognosis, while Peter walked her to John’s room. She doesn’t reach out to him now, doesn’t hold his hand or comb his sweat-slick hair from his feverish forehead. She doesn’t soothe him with comforting, loving words.
She just… sits there. Expression wooden and disinterested. And her eyes keep looking at the door.
