Summary
Six months after the Krang invaded the turtles are trying to recover from both the physical and mental trauma they had to go through. Each brother has their own obstacle to overcome. But no one is struggling harder to get over it than Leo. He's alive, he's home. But maybe it was something he didn't want. As he pulls further and further away from his brothers trying to make amends for his past mistakes, he finds his world tipped upside down once again when someone shows up asking for help. It might be inconvenient, but it might wind up being exactly what Leo needs.

“Hello?” Leo called out stiffly after a moment. His voice seemed to carry and burn out like pieces of ember. “Is there anyone there?”
There was movement in the shadows. A big, lumbering figure that when Leo saw it, he couldn't believe he didn't notice it. He immediately placed a hand on one of his katanas and got up in a crouch. Instinctively, he put a hand up in front of Michelangelo to protect him. “Who are you?”
“Who are you?” A voice called out from behind them. Leo snapped his head to see where that voice came from and there was another figure.

A fight between Leonardo and Donatello causes an accident in their lab - and this causes Leonardo and Michelangelo to swap places with their alternate-dimensional counterparts.
The relationships between the four brothers are already strained from the death of their father, and these weird and different mystic turtles are not making it any better. Or are they? The brothers suffered a loss they shouldn't have, and they are broken. But they will be whole again - they have to. Even if that means accepting help from across the multiverse.

After Splinter announces that Leo is now the leader, everything quickly moves downhill. Raph won’t speak to Leo, Donnie snarks and pokes and prods at him and Mikey comforts the other two all while avoiding Leo, because apparently, he didn’t need help. They all……blamed him. He didn’t want this, he’d NEVER wanted this, but now it was his fault?! When he confronts Splinter about it, the old rat just waved him off, saying “You’ll get over it Blue, now MOVE, you’re blocking the TV!!!” which honestly made Leo realize how much Splinter had done that growing up, and that it probably wasn’t okay.
(Or)
Leo has enough and decides to leave. Que the Battle Nexus, Big Mama, Hueso, and getting to know Villains! What happens when, about four months later, Donnie makes a portal and brings through their alternates, but realizes he needs Leo’s help? Chaos!
(I changed up the timeline of Splinter making Leo Leader. Why? Plot Convenience.)

Mikey pulled through for them, though. Somehow he managed to open a portal to the Prison Dimension. Raph had Leo in his grasp, pulling him in, so what happened?
How did they get here? And where exactly was 'here' anyway? He just wants to protect his brothers and go home. After everything they've been through, it's the least the universe owes them.
