Cas had one hand curled over his stomach, one grinding against his forehead, and his eyes were darting everywhere. Dean couldn’t catch his gaze. “I n-need to find—I need to get—safe.”
“Let me help. It’s me, Cas. It’s me.”
Something in Cas’s expression calmed, for a moment, but then he looked like he was on the verge of tears. “It’s me, we’re family,” he said, eyes finally looking at Dean straight-on, intense.

When Dean nearly runs over an injured Castiel on the way back from meeting Metratron at the end of 08x21, it doesn't take long to realize something is seriously wrong with his friend. More than he'd realized, more than Cas can hide. He can't speak, can't stand, can't remember things. Dean will have to swallow the bitter feelings he's been harboring since the crypt and help him, or Cas might not make it through the night. But if he does pull through, maybe Dean will get another chance to make Cas understand what he's been trying to say.

“I wish I could heal you,” Cas says, pained.
'You already do, Cas', Dean thinks. “It’s okay, bud,” he slurs instead. Feeling warm and sleepy lowers all of his guards and, as true exhaustion begins to creep up on him, so does an unsettling feeling that Dean doesn’t know how to explain. It’s like Dean and Cas aren’t alone in the room, someone else watching and breathing down Dean’s neck from the shadows of his dreams.
“Cas,” Dean breathes, a tight feeling in his chest. “What if he’s still here?”

(Dean kicks Michael out of his body, but continues to be haunted by him---his Grace and his voice poisoning Dean's body and mind, corrupting the very core of his self. “You think I just walked backwards into a nail?” Dean finally asks, voice low and dangerous. “After everything. After Michael and Lucifer and Chuck. You think I let a nail do me in?”
“You wanted to die?” Cas tips his head to the side, eyelids shuttering, mouth drawing tight. Just like the first night they met. It’s nostalgic, in a way. Dean’s half tempted to run him through for old time’s sake.
“If you were out of the Empty,” Dean says loudly, talking over him, “then you’d come down and save me, right? And if you weren’t, then I figured Heaven was as good a place as any to get a rescue started. But I guess I overestimated you, Cas. ‘Cause I’m dead, and you’re right here. So talk me through it, okay?”

“Yes.” Jack is nodding, smiling, all gap-toothed and kindergarten positivity.
“And the only parts of us the Empty would see are whatever dead leaves the angels and demons left in our gutters?” Dean raises his eyebrows.
“Exactly.”
He lets out a low whistle. There’s a living warmth finally coming back into his complexion. Not done yet, Winchester, still one more rescue mission. “It’s gotta be me. I’m going.”

Summary
Summary: When Cas arrives at the Bunker, clearly ill, and keeps having trouble remembering Dean or Sam or English, the brothers have to resort to a spell which paints Cas in sigils in an effort to anchor him to his own memories.
The brothers aren’t prepared to experience Cas’ memories for themselves. Through events they thought they knew and ones they didn’t know existed, Sam and Dean will have to face the fact that with the years between them, and even with Dean secretly learning Enochian, they still haven’t fully translated Castiel.
If this works, they will have Cas with them and a whole new understanding, but the full extent of what it means to be Cas might be more than Dean can handle.

Because that’s definitely Jimmy’s body slumped over against the trunk of one of the apple trees, but floating above him is- well, it’s a floating light, a sphere with a one-foot diameter, just hanging out at Dean’s chest level. Dean approaches it carefully with a furrowed brow and half-raised hackles.
“Cas?” Dean prompts slowly. “Is that-”
And the orb is glowing out sweet and white like it could easily be used to keep away the dark in a child’s nursery. It’s so welcoming. Like Dean could just reach out, out, and catch that gentleness in his hands without any sort of consequence.
“Hello, Dean,” the orb hums.

This is easy when Sam is around. When Sam is asleep in the passenger seat, and Dean can let his eyes drift up to the rear-view mirror, until his eyes meet Castiel’s in the back-seat reflection, in silent recognition of whatever they had just done in some motel bathroom. In quiet anticipation of what they would do, what they could do, hidden away in some of corner of wherever it was they were going. This is easy, when the only time they can spend alone is spent with their mouths and hands all over one another.
And now Sam is already 700 miles away and there’s nothing on the radio but static. “So, I guess it’s been years, since we did this, just you and me,” Dean says, under the shadow of a highway overpass. “Gone on a case together, I mean.”
