This is where the misunderstanding happens.
Trinity, in all of her panic and anguish, took the question to mean ‘are you okay’, so she very quickly blurted out “yeah.”
Al-Hashimi had not meant to ask if she was fine.; Trinity Santos has had an utterly exhausting, grueling day. Langdon's return, combined with all other factors, is just entirely overwhelming for a woman who feels very miserably and viscerally unseen and unwanted. She now sits in the bathroom with her pants around her ankles and her underwear still on, a sharp thing in her hand, and once she's done the woman who finds her is her worst-case scenario. Baran is not as horrible as she thinks she might be.

“So–” Dennis starts, and Trinity honestly isn’t sure how she’s going to explain this one.
“I know it’s weird,” she says, gripping the steering wheel.
“I mean, it’s not that weird,” Dennis placates.
“He’s kind of a trainwreck,” Trinity defends, and then feels some type of way about the fact that she’s defending Langdon.
Dennis doesn’t disagree.
She promises him, “It’s only temporary.”
(Langdon's wife kicks him out. Trinity offers up her couch. She ends up learning a few things about her sworn enemy in the process.)

The good thing about working in such a chaotic environment is that no one notices when you’ve skipped meals. Langdon’s always eaten like a bird, was always teased for it by family and friends. He prefers quick snacks to big meals, and that’s how it’s always been. That’s why he works so well in the Pitt. He shoves a protein bar in his scrub pocket, knowing full well he won’t eat it later. But it makes him feel like his old self, the one before the benzos, when he pretends he still has an appetite.
Langdon has a problem with food.

She won't be calling anyone. That's sort of the point. “Thanks.”
There's a finality, now. And instead of dragging her down, the knowledge that two weeks from now she'll be gone has the weight in her chest lifting. It's been so long since she's felt this that the promise of it makes her high. When she goes home tonight she won’t have to cut open her thighs, because the joy of dying, of leaving all this behind, of the decision that she’s finally made is the only stimulation she needs. There’s no punishment—this is her punishment. And she won’t be here long enough to do anything about it.

Everything after that happens too fast to think and slow enough that Trinity will replay it later in punishing detail, her brain an endless loop of cause and effect. The patient wrenches his other wrist free, the restraints tied improperly at its connection point to the bed—fucking newbies. He seizes the opportunity immediately. Trinity would notate that point on her neuro exam (patient alert) if she weren't more focused on Yolanda at her side, in optimal reaching distance.
The angle isn't great. Yolanda is bent over him and she sees movement in her periphery a fraction too late, eyes widening as Trinity calls, "Watch out!"
