As for Dottore, he truly didn't have any idea where it all really began. And perhaps even the aforementioned admirer was not cognizant of the shift in intention. Perhaps it was not something that could be recorded down to the exact time or day; a phenomenon that existed beyond the vacuum of a single moment, still shifting and evolving even long after all these moments passed.
But for the matter of simplicity, as far as Dottore was concerned, it all began that day in his office - with what should have been a knock on his door.

“You’ll be travelling with Scaramouche for the next while,” said Omega.
A decision made with neither Zandik’s consent nor input. A tale as old as time for the ‘youngest’ segment (technically he was over a century old, barely younger than some of the other segments, but that hadn’t stopped them from using his youthful appearance to justify treating him like a child).
Scaramouche decides it would be a fun exercise to rid the youngest Segment of his milk teeth.

Summary
Dottore wasn’t a sadist, exactly. He didn’t delight in other people's pain. It was just a necessary component of many of his experiments, and he was impervious to the pleas and cries of his specimens in a way that could be mistaken for sadism rather than a complete dismissal of the feelings of others. Still, it was a more believable label than 'masochist', so Scaramouche was stunned to discover that was the category Dottore fell into.
Scaramouche lashes out at Dottore and discovers something interesting about him.

Are all the different segments of the Doctor merely cheap imitations, clones of the original? Or by extraction of these parts, have they become separate individuals? Or perhaps they are all parts of the collective self that make the Doctor.
"What is it that makes an identity and person who they are?" Dottore posed the question to the segments at the rectangular metal table.
"The answer," responded Dottore, "must lie in those we love."
Dottore scoffed at the sheer sentimentality. A preposterous notion, a weak emotion. Ill-suited for the rational mind, and for inflated fools to praise themselves along with their fragile egos. The other Doctor only shook his head at the utter absurdity he was displaying.

Dottore’s brow arched faintly. “Adorable?” the word seemed foreign on his tongue. Before he could reply, Pantalone extended a gloved hand.
“Come, indulge me with a dance. No one watches but the stars, and they do not judge.”

As the Fatui's schemes progress towards the point of no return, Childe grows concerned with the Second Harbinger's suspicious behavior. Having a personal investment in the matter, he decides to do some investigating on his own. The Eleventh is no stranger to manhunts, but uncovering Dottore's true intentions will not be a simple task. How does one track down a man who is simultaneously everywhere and nowhere at once? And can one even be sure that they've found what they're looking for when they see it, in a situation where it's so easy to lose sight of the forest for the trees? Only one thing is immediately certain; the Doctor's many Segments certainly don't intend to make it easy for him.

Perhaps not. But Dottore can see the hunger enclosed in Pantalone’s eyes. Endless and insatiable, it devours the violet of his irises, swelling out from the yawning maws of ink-dark pupils.
Dottore’s voice pitches low. “And what is it you want, Regrator?”

Going to Dottore for help was the last thing Childe wanted to do before his long mission, but the doctor is the only person he can think of that may have a medication like the kind he needs. When he discovers secrets within the lab that are intimidating by sight and sinful by nature, Childe can't help the ways his eyes keep drifting to the machinery Dottore has created that fail to resemble the type meant for warfare.
_____Dottore leaned back as he raised an eyebrow. “If you do not stop asking questions, I will start to think you have reasons for wanting to stay.” Crossing one leg over a knee, he gave a mock look of wonder. “Are you perhaps a little curious? Is that it? Do you just not know how to use your words and ask?” One hand supported his chin while another drummed on the arm of his chair. Childe's gaze met his, and Dottore smiled. That little wicked grin glinted with amusement, sharp canines revealing themselves as he chuckled.

A hand cups his chin. At some point, it seems Dottore has removed his gloves. When did he remove his gloves? Pantalone feels the scars scrape over his jaw.
“You always did overestimate your ability to lie to me.”
Pantalone grieves the aftermath of Dottore’s encounter with a God. Under the watchful stars, they reconcile.
