The Delusion had not been kind to Diluc's body.
Nearly four years he had wielded the accursed weapon — the same gem responsible for killing his father, alongside the knife that Diluc used to end his misery — and for nearly four years, it had only deteriorated his body. Combined with being so far away from his Vision, and well — it had been a miracle that he remembered who Kaeya or his father at all.
Nearly four years that he had managed to keep the secret under wraps; the matter that the weapon had done irreversible damage to his body, both physically and mind. Despite Kaeya's insistence, cigarettes really were the least of Diluc's health issues.
OR; Diluc is suddenly gravely ill, prompting a crisis response from those closest to him. Kaeya discovers long hidden secrets within the family in the process — ones that, in his opinion, should have remained buried.

"Y'know what your problem is, D'luc?" Kaeya pointed both his finger and his sloshing, half-drunk glass in the other's direction. Wine dribbled over the rim, splashing onto the wooden countertop. The sight of a wasted sip had Kaeya pouting like a baby.
There was a grunt from behind the bar. "No... but you'r going t'tell me anyway, aren t'you." Despite sounding somewhat proper, with his usual gruff intonation, Diluc's words were still a little slurred.
"Your probl'm is, you'r too uptight... You shoul' relax more off'en... Have fun. Isn't it fun, drink'ng with your baby br'ther?"

Diluc braced himself, holding his breath as Freminete picked up the shotgun with trembling hands. The silence was deafening; it made Diluc's turn as he waited for his fate, staring down the barrel as Freminet closed his eyes. "Look at me," Diluc ordered in a gentle voice, squaring back his shoulders.
"I can't," Freminet whispered pathetically, his finger hovering over the trigger as he aimed the gun at Diluc's chest. "I can't shoot you..."
"Failure to comply with the rules will result in both your death and your brother's," the voice reminded Freminet.
The young boy swallowed hard, raising the gun up again and hovering his finger over the trigger. he couldn't bring himself to look at Diluc as he whispered, "I'm sorry," and pulled the trigger.
In order to get Kaeya back, Diluc has to play a game with the Fatui. Too bad the Fatui have a sick and twisted sense of humor.

Summary
Routines are something small. Something that makes him feel in control of his life. Diluc is used to live in a certain way. Changes are rare and he has a hard time adjusting to said occasional changes.
But what'll happen when Kaeya suddenly starts avoiding him? Kaeya is part of his schedule, they have a script and Kaeya isn't following it. It's distressing and out of character.
Kaeya is simply trying to let go of their broken relationship after trying to mend their past connections for the last few years. Diluc wasn't making any effort and it was frustating for the blunette to see the way he kept clinging to their past like a leech.
Diluc does not take this well.
