Unfortunately, the relapses from John's death and Dean's trip to Hell were bad, but they had nothing on the downward spiral triggered by Sam starting the Apocalypse.
But Sam has never come clean before, and he isn't about to start. He's self-medicating, he's suppressing, and he's handling things in the Trademark Winchester Way. He can do this. He can totally do this.

Summary
God is dead. Sammy finally has what he always wanted. The kid is alive again and trying to figure out the whole human schtick. Cas – Cas is complicated, like always. And Dean is barreling headfirst into a mental breakdown. It’s the end of the road so far, and the future never looked so frightening.
“I can’t,” Dean stammers into Cas’s chest. “With you – I can’t.” And Cas goes very still, stops rubbing his hand up and down Dean’s back. But I want to. Please, Cas, don’t leave me. Because I want to.
“Dean,” Cas says, still not moving. “I can tell you’re praying to me, but I can’t hear you. I’m sorry.”
An alternate ending to the series, focusing a lot on *feelings* and *mental health* and *processing trauma* – because, you know, what’s better than that?

He’s not going to tell his little brother about the handful of benzos and oxycodone still churning in his system, because Sammy’s breathing hard through the gunshot in his gut, and Dean’s not going to add to Sam’s already extensive list of problems. And Dean knows how Sammy gets whenever Dean tries to trade his life in for his little brother’s. It’s never been something Sam takes well. So Dean isn’t going to tell him.

It’s okay during the day—well no, it’s not okay. Sam will clap Dean on the back or shoulder, Jack will brush against his arm when they’re standing side by side at the stove. And what used to feel so normal—good even—will feel like too much to handle.
Eileen and Sam will hold hands or give each other cheek kisses, and Dean will have to bear feeling so happy for his brother and yet so jealous at the same time. That’s just it, during the day, Dean’s distracted and has reason to keep a tight lid on his crap. But at night—
He keeps himself awake until he absolutely can’t keep his eyes open anymore. Then he lies alone in his bed in the dark, and the monster comes—like a hermit crab, it tries to claw its way out of its shell, out of him. It knows nothing but want, and fear of what it wants.
Unable to stand it, Dean’s fingers will curl into the sheets and his heart will race and his throat will feel like there’s a scream lodged in it. Not even his old trick with the hot water bottle can lull his body into believing it's not alone. It’s not working anymore, not since his body got a taste of the real deal. Not since he’s been hugged and kissed and his hand has been held.

The man standing in front of him was definitely a clone, alright. His hair was surprisingly shorter than Grace’s had been when he woke up. Maybe he found the scissors already. He had something closer to a beard than stubble. His nose was more crooked than Grace’s. He was wearing some of Grace’s clothes, frankly looking ridiculous in a Cats t-shirt and red jumpsuit tied at his waist.
His expression was surprisingly neutral, hands raised slightly to signify he wasn’t dangerous. “Ry?”
Grace probably looked stupid, mouth opening and closing like a fish.
“It’s me, Colt. Do you remember me?” Colt shifted his weight between his legs, nervous in a way that didn’t seem natural to a man like him.
The words on Grace’s tongue died out. His heart beat faster, his brain reaching for a memory. One second he’s on the Hail Mary, the next he’s back on Earth.

Canon Divergent after 10x03: Soul Survivor. After being cured, Dean struggles with the emotional pain and the guilt he hadn't been able to feel while he was a demon. He's scared the Mark will take control of him again and deliberately lets himself get injured during cases because he feels like the pain helps him fight the Mark's influence. That doesn't work out indefinitely though, and it's only when Dean finally breaks down under the weight of it all that Sam and Cas notice something is seriously wrong.

It took Sam a while to notice it. Longer than he’d like to admit. But to be fair to himself, he had been watching for impulses of a more murder-the-world kind, not whatever was going on now.
It all really boils down to this: Dean styles his hair. Which Dean has always done, and Sam has always known. But the hair gel is no longer tucked away in the bottom of his duffel bag; it stays out in the open of the shared bathroom of the Bunker, and Dean doesn’t hide what he’s doing if Sam walks in.
“Heya Sammy,” Dean greets happily, running a hand through his hair.
“Hi,” he says back, nervously watching Dean’s actions.
He’s looking in the mirror, and it’s not until he stopped that Sam realizes Dean’s always flinched at his reflection.
