Leo’s confident persona sagged, and Donnie steeled himself for an interaction far outside his comfort zone. “So…” He sighed. “Care to have a conversation about your feelings with a brother who most definitely understands them, he said, lying through his teeth?”
Leo rubbed one hand against his bad knee. “It’s really stupid,” he said.
“Well,” Donnie said, “as unfamiliar as I am with things of that nature, I shall do my best to hear you out.”
No response at all to the quip. Something was definitely wrong.

Or: One month after the invasion, Donnie wakes up to a scream coming from Leo's room. “Leo isn’t at a station,” Donnie's automated voice said. “He’s on the tracks.”
Raph frowned. “What? Why would he—?”
He felt Mikey’s realization just as it hit him. Do everyone a favor and just let me go. This wasn’t Leo going off to sulk on his own… this was much, much worse.
Mikey pressed his hands against his mouth, tears welling in his eyes. He looked to Raph, who extended an arm and pulled him into a hug. Raph could barely think. He hadn’t expected to get through to Leo in their conversation, but he’d pictured finding Leo at Hueso’s or Bros’ Pizza, somewhere distant but safe. Had what he’d said pushed his brother that far?
Mikey finally spoke with a shaking voice. “Isn’t there a third rail there?”
Donnie nodded.
That knowledge hung in the air for a minute. Once again, Leo had gone through a portal with no intention of coming back.

Or: As the Hamatos struggle to find a new normal following the invasion, Leo begins withdrawing from his brothers. After it becomes clear that Leo's problems go far deeper than mood swings and refusing food, the rest of the team has to set aside their emotions in time to find Leo before it's too late.

Summary
Donnie collided with Leo in a tangle of limbs. There was an instinctive cling in return from his twin, before the realization sunk in.
"No." Leo breathed in his ear, immediately shattered. "No, no, no, no, no!”
Every muscle in Donnie's body was shaking like an earthquake from the amount of effort he'd just expended. The explosion caught up to them, sending the twins spiralling away into space and debris.
For one singular moment, Donnie allowed himself a burst of victory. His heart was still thudding a thousand miles an hour in his chest, having been absolutely terrified he wasn't going to make it in time. But he did. And he held onto Leo, his prize.
Then they hit the ground, the sudden tumble bursting them apart. Donnie felt the world spin, rapid and disorientating, before he slammed into something rock and cracking under his impact.

The portal closed, cutting the Technodrome in half and ripping away Leonardo from his family. The hit Mikey took to the head left him unaware of the loss until well after they returned home, one less brother following behind them. Mikey wasn't one to give up on his siblings though.
In the Prison Dimension, Leo is visited by a familiar power that aids in his survival at a high cost.

Donnie’s head pounded and his limbs felt like wet paper as his senses started to return. Even with fuzzy vision, he could make out a horribly familiar lavish office. Through the large, floor-to-ceiling window the rising sun shined directly in his eyes. His ringing ears made it hard to distinguish sounds, but he recognized the honeyed, singsong voice emanating from the lavender blob in front of him anyway.
“Good morning, turtley-boos! I have a business proposition for you.”
