In a quiet place in Dean Winchester’s heart, a flame roared. Its flickering light was born of kindness, of some nameless, ageless love, of affections he found he couldn’t speak but felt dearly all the same. Of his love for Cas.
But even though Cas was all Dean had ever wanted, even though Cas was the one who his gaze caught on and his hands brushed over and his chest throbbed for, he knew that nothing would come of it. Nothing could.But Cas was an angel.
Angels didn’t feel love.
But then, over afternoon research on a rainy Thursday that was otherwise completely ordinary, Cas said, “We should get married.”

“So, what now?” The angel’s voice was deep and steady as always. Dean didn’t hesitate like he thought he would. He didn’t think of an excuse to change the subject or avoid Castiel. He didn’t think of a reason that he could isolate himself and set himself down a path of self-destruction.
“I made a promise to Sam. If you’re up for it, and I understand if you aren’t, I know there’s a lot better things for you out there than a broken human, but if you are then--Cas, do you want to hunt with me?” The angel smiled.
“Yes, Dean. I’ll hunt with you. If you want me, if you’ll let me, I’ll follow you into the end of the world a million times over.”
Dean smiled sadly back at him. “I’ve said it before, Cas, but don’t ever change.”
OR
Instead of finding Lisa after Sam dies in Swan Song, Dean and Cas hunt together.

Summary
It’s easy, after that, to call Cas when he is like this.
When the numbing is back and when the pain won’t leave him.
It’s easier than breathing, calling Cas and waiting for him like he is the remedy to his sickness. To his problems. To his tortures.
Castiel doesn’t ask him anything he doesn’t want to be asked and at first, Dean is grateful.
And when Cas stays there, holding him throughout the night and not leaving until the morning, he doesn’t say that he appreciates it.
That he wishes Cas wouldn’t go at all.

Or, Dean needs to feel something, anything, and Castiel is the only one that can help him.

Castiel smiled fondly at the familiar words, tracing random designs on the side of his glass with his fingertip. “This one right here. If I only have a short time left here before… I like quiet moments like these. With you. Whatever time is left, I’d like to share it with you. If you’ll have me.”
They both liked to play this game. Throwing out words that could have entirely different meanings. Nervous looks exchanged between the two of them, wondering if the other is going to take up the offer that’s hidden, no, intertwined within words that are balancing on the line between “brothers” or “lovers”.

Inspired by the song "If The World Was Ending" by JP Saxe and Julia Michaels. Castiel and Dean share a moment in the bunkers kitchen, in which Castiel reveals something he's been hiding from the brothers.

Which is why, when it reached out one long, dripping black hand out to him, Dean reached out, too.
Dean didn’t know if it could talk. It didn’t need to, anyway. Dean knew what it was asking, and he answered the silent request without a second thought.

Faced with death, Dean makes one last ditch effort: praying to an Angel he knows wont hear him. Deans prayers are answered when a vessel-less Castiel forces himself out from the Empty, taking possession of Dean's body in order to heal him.
Castiel's grace is running finite however, charged down after saving Dean's life. Now Castiel resides within Dean's mind, too weak to survive a transfer to another vessel, leading them to a desperate search for a way to rebuild his body.
