Is it only hope on the big scale? That the world is not so dark and depressing and destructive as the villain in front of you says it is? That you can win, even when everything else says that you can’t? That maybe it is better to live your life, even afraid, than it is to keep yourself sequestered away, alone?
Does it not also mean hope on the small scale?
Or: Makoto sacrifices himself in the hope that the other survivors might be able to help Junko. It remains to be seen whether this will actually succeed.

“Sheesh, as I was saying!” Monokuma flails his arms. “Momota-kun and Shirogane-san seem to have come down with an oldie but a goodie—the despair disease!”
Monokuma’s announcement is regarded with empty stares from the whole group, aside from Tsumugi, who pushes on Gonta’s glasses and lets out a giggle, mumbling something about how he must be honoured to get to hold her. Gonta agrees thoughtlessly, nodding his head a little, but his gaze is still solely on the robot bear.
The silence is, as it so often is, broken by Kokichi. “Ooh, the despair disease!” He bounces forward with starry eyes, a broad smile on his face. “How exciting! Hey, Monokuma, do you wanna explain what that is to these peons, or should I?”
“You really know how to take the drama outta an announcement,” Monokuma grumbles.

After the second trial, Monokuma re-introduces a familiar motive for the third chapter: The Despair Disease.
Naturally, the consequences from this are immediate, and great.

“So are you going to kill someone?”
“Sure could,” Iruma muttered, letting out a little scoff. “Could kill you even. Bet you wouldn’t be able to fight me off, babysitter girl.”
Interesting choice in nickname. And also: factually incorrect. From her position, Harukawa could see seven different ways to incapacitate Iruma, and that was without moving from her spot.
She shifted her weight. If Iruma attacked her, she would obviously defend herself… But the idea of killing to escape made her nauseous. The orphanage, inevitably, would find a way to proceed without her. More than likely, one of the older kids would be chosen to replace her, and Harukawa couldn’t stand the thought of that—but the thought of killing someone without being under orders made her feel worse. Her hands curled at her sides. She hated being indecisive. Truthfully, it had been so long since Harukawa was truly acting under her own will, she had no idea what to move.
She wouldn’t, however, kill Iruma.

“I thought you were a kindred soul, Castti,” he murmured. “I’ve overheard you talking to yourself many times before. Others have noticed it as well. It’s like you’re conversing with someone who isn’t there.”
Castti stopped and looked up to Osvald once again. Genuine confusion was written in her eyes, and she blinked at him a few times before she was able to remind herself that she needed to respond. “Really? I don’t remember ever doing something like that.” She would have remembered if she had been talking to herself. Surely people only spoke to themselves if they had an intention to do so. Perhaps Osvald had simply misheard her. That would make sense.
Osvald continued to stare at Castti as she forced herself to get back to her work. He didn’t seem to believe her, or if he did believe her, he thought there was more to it than she claimed. Either way, he left it there, seeming to see it was a pointless argument to have. After that, he just got back to watching Castti work, and all the while, she felt Osvald’s eyes against the back of her head.
