Sukuna has been kept in a constant state of rut since he's been abducted, and he just might be feral. If he is, it'll be too late to save him.
Known as “Ryomen” in the ring, he is a certified alpha killer.
Yuji and Sukuna's older brother, Choso, was also abducted the same night, but there's no trace of him, its suspected that Sukuna knows what happened to him but the alpha refuses to speak. He won't even let anyone near him.
Except Fushiguro. He is the only omega, only person at all, he will tolerate near him.
Omegas and betas did this to him, and alphas he was forced to fight for his life. No one is safe until Megumi decides they are.

Summary
Both his father and the viscount had explained it once or twice, about all the complexities of relations in society; friendships, courtships, the appropriate closeness expected of alphas, betas and omegas. Where to step, the distance, the chaperoning necessary to hold onto ones honor, omegas especially.
Any misstep over those rules would ruin marriage prospects and reputation alike. An omega in search of a match would heed the alpha's seemingly kind warning and leave quickly, never to repeat the same mistake again.
Megumi was not that omega.
"Well, if I come across an omega who is in search of a match, I will tell them to refrain from activities such as these," He put Kon on the ground, taking his leash again, "As it stands, there is nobody around that cares for your warnings."
The alpha seemed confused, "Did you not say you were of age? Are you perchance married yet? Or just engaged?"
"I am neither, good sir." Megumi turned around and started walking away, "I am merely a spinster in a walk with his dog."

In the late Heian Era of Japan, Yuuji, along with his twin brother Sukuna, is an alpha imperial prince, son of Empress Kenjaku. Now that the twins are sixteen, it is customary for alpha nobles to pick a mate from an omega repository—a place where presented omegas, rare as they are, are kept and saved for nobles only. At one of the repositories, Yuuji meets Nanami Kento, an omega with stereotypically "undesirable" traits who is almost at the age where he will no longer be fertile, and therefore will be sent to an omega den—a brothel where older omegas are whored out to alpha peasants (and often treated awfully) every night for the rest of their lives. Feeling bad for the older omega, Yuuji chooses him as his mate just to spare him from that fate. Over time though, the two begin to develop genuine feelings for one another. A wrench is thrown into their budding love though, when Kenjaku gives a deadline for Nanami to get pregnant with an heir; otherwise he will be sent to a den and Yuuji will need to go with another mate. All of this takes place on a backdrop of a war against anti-imperial rebels, Sukuna being forced to participate in said war, and a beautiful young noble omega named Megumi causing a rift between the twins.

He had never wanted the typical omega/alpha bond, his scent blockers did a good job of keeping his scent hidden, and pretending to be uninterested in alphas wasn’t exactly difficult to do when he genuinely had never liked one.
That was until a dorm mix up put him in Dorm 43 with Ryoumen Sukuna, an alpha who is arrogant, irritating, and is unfortunately the only one Megumi has ever liked the scent of.

Sukuna chuckled, low and indulgent, as if Megumi had missed a joke. "As my bride." The declaration rolled off his tongue like a vow, thick with possession. One of his lower arms slid up Megumi’s back, claws retracting as he palmed the nape of his neck with grotesque tenderness. "You’ll want for nothing. No more battles, no more losses." His thumb brushed the hollow of Megumi’s throat. "Just my attention. My... devotion."
