Summary
Tommy had suffered on and off throughout his life from hanahaki, which sort of happens to someone who trusts too much and always gets their hopes crushed. He just wants someone to actually care about him, but he knows he's not going to get that, especially not with Dream and not with how the disease has been getting worse.
Dying and defeated, Tommy's only solace comes in the form of weird eldritch singing emanating from the forest outside his window. He knows that whatever it is will likely be a cannibalistic cryptid from his worst nightmares, but somehow he can't find it in himself to care. Any death will be better than suffocating alone.
Or
Tommy is on a quest to get killed by unknown monsters instead of the ones he does know. This is somehow more difficult than it should be.

Dream is still there, still a threat, still banishing his sword, but the threat of swords crumble in the threat of obsidian wings. And the threat of obsidian wings crumbles in the threat of another sword being unsheathed and a gait too heavy to be any of the people from L’Manburg that come from behind Tubbo.
“He is not the only one,” a voice rumbles from behind—deep, echoing, and terrifying. “He’s just the flashiest.”
Tubbo looks up and wishes he’s surprised to meet the eyes of the Blood God.
Or: The SMP has no idea exactly who Tommy and Wilbur’s family is.
So imagine their surprise when—after Wilbur enlists his family to come help finish the war between L’Manburg and the Greater Dream SMP—it’s not another scrappy nobody with a too loud mouth and too many dreams that shows up in the midst of war. No, instead, Wilbur and Tommy’s family happen to be Technoblade, the Blood God, and Philza fucking Minecraft.

It was a Wednesday.

Those friends are also superheros and vigilantes and the occasional supervillain, and all of them seem to love giving him a heart attack by showing up at his home at irregular times occasionally bloody from head to toe. (And also cleaning out his fridge every time they visit. Why.)Or, in an alternate universe, Technoblade's just a guy with friends that can fly, move things at light speed, and scream real loud and break all of his windows as a demonstration that no one asked for. Wilbur wove through the crowds, aiming for some destination that Tommy didn’t know about. He paused for a second, deciding on some turn, and the horrifying sound of an arrow sinking into flesh sounded dangerously close to Tommy.
He whipped his head around, horror growing when he saw the body, crumpled to the ground, an arrow sticking out of her throat. She’d run in front of Tommy by accident. Wilbur saw the arrow too, and he snarled.
“Tommy, listen to me.” He released Tommy’s hand to grasp his face. “It’s basically a straight shot from here to the fountain, Dad can help you get there. I’ll be right behind you,” he promised, then shoved Tommy in the direction of the fountain, standing his ground.
