The title is self-explanatory, but in case it isn't—Homelander finds out about Butcher's tumor, and when he scans down Butcher's already decaying body one last time before he leaves with Ryan, he notices something out of place: his breasts are abnormally enlarged.
It vaguely reminds him of his stalker days with Madelyn, when she was trying to breastfeed little Teddy.
Or Homelander hyperfixates on Butcher's condition long enough to understand that his tumor is destroying his testosterone levels and raising his prolactin high enough to possibly trigger lactation with the right stimulation—something he can't quite stop thinking about once the idea settles in.
And once it does, everything gets out of hand.

He had done it a dozen more times after that. No longer at Club Rouge, but at bars and clubs that were further away so he wouldn’t run into any friends. Any friends except Amy, once. Which turned into a long lecture on safety and the dangers of being a woman at a club before she was swapping pretty outfits with him for his next nightly adventure. He would smile sweetly at strangers and wouldn’t mind when a hand pressed against his lower back to lead him somewhere, or when a man insisted they take him home. He liked it– no. He loved it. Being the center of their attention was dizzying and he couldn’t get enough of it. Being able to tease a man while they showered him in compliments and drinks before he slipped away for the night was nothing short of thrilling. He couldn’t stop; he wouldn’t even allow himself enough time to think of the repercussions of flirting with the wrong man as he delightfully accepted another drink.
