Inej will never tolerate a man looming over her in a bed again, and Kaz cannot look down at a body without being reminded of bloated, marred flesh. And by accommodating one of those fears, they can accommodate the other.
They easily come to the joint conclusion that Inej should simply endeavor to be above Kaz whenever they find themselves wanting more than casual touch.

"He usually didn’t drink much (almost never, in fact), but the Dregs had finally won over a bit of extra territory that he had become personally invested in. It was rare now for him to care so much about a mere territory dispute, but then he had been stabbed by a Razorgull, and the scar had become so itchy that he decided to make an exception. And Kaz was far from the only Dreg sporting itchy scars, so it wasn't long before celebratory drinks had turned into something far more unruly.
Kaz wasn't sure why he joined this time, but when Jesper offered, he'd agreed within seconds. Luckily, it only took two and a half drinks for him to realize that a large part of him was exhausted of fighting against the things that might make him... happy. Not because he was the sort of person who deserved happiness, and not because kruge couldn't make him happy anymore (that really would never change). It was because this was the first time in a long time that Kaz actually believed it was a legitimate possibility."
Or, Kaz tries to touch Inej and fails, so they invite Jesper and have a threesome instead.
