When Caesar finds the ibuprofen he senses the scent of an omega going into heat, how? There's no omega inside his apartment, it comes from the living room where Joseph currently is. Quickly grabbing the pain meds, in haste Caesar heads into the living room, shocked at the sight of the scent coming from Joseph.
Joseph looks up at Caesar holding his hand out, panting heavily "It's getting worse, I might have a fever or a flu"
Caesar swallows, the scent of him is so good. It doesn't look like Joseph realizes he went into a heat, it must be his first heat. Looking down at the ibuprofen, it's not going to help Joseph with his heat. It has no effect on heats, placing the box on the kitchen table. Caesar tries to ground himself, not to act on his one sided crush and claim Joseph as his own.
